IN-HOME FAMILY SERVICES

Outcomes, Inc. provides In-home and family services to children, adolescents and adults in Chisago, Isanti, Washington, and Ramsey County. Our services provide the following:

Independent living skills
Individualized programing
Self Advocacy
Assistance with homework and tutoring
Social activities, Community and Peer integration
Assistance with budgeting
Goal setting
Assistance with doctor and therapy appointments
Assistance with health living habits
Coordination services
Behavioral assessments and programming
IEP/other agency meeting attendance
Respite and Waivered services
Resume development, interview skills, job performance evaluations and on the job training
Assistance with transportation

RESIDENTIAL SERVICES

Outcomes, Inc. provides residential services to children, adolescents and adults in three homes in Ramsey County.
We assist individuals in our residential services with:

Daily Living Skills: helping to develop and increase independence
Individualized Positive Behavior supports: programming that incorporates best practices and techniques to enhance success
Functional Communication is used to increase choice and independence
Adaptive Behaviors are identified to meet consumer needs
Relationship building through participation in social events, increasing involvement with family and friends
Everyone has the right to participate in development of their own treatment plans, we encourage choice and independence in setting up plans
-Programming is designed to increase skills and accomplish self-determination, dignity and quality of life.
Specialized Programming uses supports and teaching methods that accommodate each individual
Programming incorporates meaningful and natural community participation
Sensory Accommodations developed for individual needs

Outcomes, Inc. is governed by a volunteer Board of Directors which includes parents and community members.

Among our board members, we have the former CFO of Bremer Bank Holding Corporation, The former Executive Director the Autism Society of Minnesota and the Developmental Coordinator of the Twin Cities United Theological Seminary.

Our board of directors provides a wealth of experience and technical assistance to support our mission and philosophy.
